16.14 Behandlung von Harninkontinenz
Dieses Kapitel hat die Behandlung der weiblichen Inkontinenz zum Gegenstand. Es stellt Ihnen die auf

Bei der Blase werden zwei Arten von Phasen unterschieden: kurze Phasen mit willentlicher Ausscheidung
von Urin, getrennt durch lange Phasen, in denen sich die Blase allmählich füllt.
Die Kontinenz, d. h. die Abwesenheit einer Urinabsonderung während der Füllphase, erfordert einerseits
eine entspannte Blase und andererseits eine permanente Schließung des Harnröhrenschließmuskels. Die
Störung eines dieser beiden Elemente hat eine Harninkontinenz zur Folge. Klinisch werden drei Arten der
Inkontinenz unterschieden:
1 Dranginkontinenz:
Die Blase kontrahiert anormal (Überaktivität des Detrusors) und drückt auf den Urin, wodurch der Druck in
der Blase steigt.
2 Stressinkontinenz:
Der Harnröhrenschließmuskel ist in seiner Funktion gestört und kann bei einem plötzlichen erheblichen
Anstieg des abdominalen Drucks (Anstrengung, Husten usw.) nicht geschlossen bleiben.
3 Gemischte Inkontinenz (Drang- und Stressinkontinenz):
Kombination aus Drang- und Stressinkontinenz in größeren oder geringeren Anteilen.
