Testgevoeligheid en -specificiteit
In de zelfteststudie identificeerde de Rapid Antigen Test 82,5%
(CI: 67,2% - 92,7%) van de geïnfecteerde mensen en 100,0% (CI:
96,5% - 100,0%) van de niet-geïnfecteerde mensen correct. Als de
test werd uitgevoerd binnen de eerste 5 dagen na het begin van de
symptomen, identificeerde de test 86,2% (CI: 68,3% - 96,1%) van de
door een PCR-test geïdentificeerde geïnfecteerde personen correct.
Dit betekent dat van de 100 werkelijk geïnfecteerde patiënten de test
83 geïnfecteerde patiënten detecteert. Als de test wordt uitgevoerd
binnen 5 dagen na het begin van de symptomen, identificeert de test
86 van de 100 geïnfecteerde patiënten correct.
Voor monsters van patiënten genomen binnen 7 dagen na het begin
van de symptomen resulteert samengevat in de volgende gepoolde
prestatiebeoordeling van de test van de drie hierboven beschreven
studiecohorten. De test toonde een gevoeligheid van 83,3% en een
specificiteit van 99,1%.
Samenvatting van alle patiëntmonsters binnen 7 dagen na het begin
van de symptomen:
Analytische prestaties
1. Detectielimiet (LoD)
Het SARS-CoV-2-positieve monster werd verkregen door een met
een PCR-test bevestigd SARS-CoV-2-negatief neusuitstrijkje toe te
voegen aan geïnactiveerd SARS-CoV-2 (2019-nCOV), stam NCCP
43326/2020/Korea. De LoD werd bepaald door het testen van een
verdunningsreeks van kunstmatige positieve monsters als 9,25 x
101,2 TCID50/ml voor directe neusuitstrijkjes.
2. Kruisreactiviteit en microbiële interferentie
Er werd geen kruisreactiviteit of interferentie gevonden met de
volgende micro-organismen bij de vermelde concentraties:
Menselijk coronavirus 229E (1 x 105,5 TCID50/ml), menselijk
coronavirus OC43 (1 x 107,77 TCID50/ml), menselijk coronavirus
NL63 (1 x 105,07 TCID50/ml), MERS-Coronavirus (4,17 x 105 TCID50/

2 (1,15 x 107 TCID50/ml), Adenovirus type 5 (1 x 107,53 TCID50/
ml), Adenovirus type 6 (1 x 107,29 TCID50/ml), Adenovirus type 7A
( 1 x 105,15 TCID50/ml), Adenovirus type 11 (1 x 107,29 TCID50/

Kruisreactiviteit voor SARS-CoV is waargenomen.
Opmerking: Op menselijk coronavirus HKU1, Pneumocystis
jirovecii (PJP) en Mycobacterium tuberculosis werd niet getest.
Een kruisreactie met het menselijke coronavirus HKU1, PJP of
TB is mogelijk, hoewel de procentuele overeenkomst van de
nucleocapside-eiwitsequentie van HKU1 en de eiwitten van PJP en
TB met de nucleocapside-eiwitsequentie van SARS-CoV-2 31,6%,
12,3 % en 13,0% bedraagt, wat als gering wordt beschouwd.
3. Onderzoek naar exogene/endogene storende stoffen
Bij de genoemde concentraties werd geen interferentie gevonden
met de volgende stoffen:
Chloraseptic (menthol/benzocaïne) (1,5 mg/ml), Naso GEL
(NeilMed) (5% v/v), CVS Health neusdruppels (fenylefrine) (15% v/v),
Afrin (oxymetazoline) (15% v/v) v), CVS Health Oxymetazoline (15%
v/v), CVS Health Neusspray (Cromolyne) (15% v/v), Zicam (5% v/v),
homeopathisch middel (Alkalol) (1:10 verdunning), fenolspray tegen
keelpijn (15% v/v), tobramycine (4 pg/ml), mupirocine (10 mg/ml),
CVS Health fluticasonpropionaat (5% v/v), Tamiflu (oseltamivirfosfaat)
(5 mg/ml), volledig bloed (4%), mucine (0,5%).
4. High-Dose-Hook-Effect:
SARS-CoV-2-viruskweek werd aan monsters toegevoegd. De
SARS-CoV-2-viruskweek vertoonde geen Hook-effect tot 1 x 106,2
TCID50/ml.
Om de grens tussen het hele-getal-deel en het fractionele deel
van een getal aan te geven, wordt in dit methodeblad altijd een
punt als decimaal scheidingsteken gebruikt. Er worden geen
scheidingstekens voor duizendtallen gebruikt.
