Please note:
The following measures are still in place at the vaccination centres:
Keep 1.5 metres away from other people.
Wear a face mask. You must bring the face mask yourself.
You will be given the injection in your upper arm. Wear clothing that lets you bare your upper arm easily.
What should you bring?
You can only get your vaccination if you bring the following things with you:
Your passport, driving licence or identity card.

If you have been invited by your medical specialist or by your employer: your invitation letter.
A completed health questionnaire. You received the questionnaire with your invitation. Did you not bring the
questionnaire? Or do you want it in a different language? Then download the questionnaire here.
Your vaccine
You will be vaccinated with Vaccinatie herhaalprik mRNA. For more information about how the vaccines work and how
effective they are, please visit the Dutch government website (in Dutch only).
Getting to the vaccination centre
You can use Google Maps to get directions to the vaccination centre. For some centres it is useful to know more about the
route or centre before you go. Details you need to know about the route and centre are given at the end of this message.
Important: Come to your appointment alone.
It is best not to bring children or other family members with you to the
vaccination centre. If it is really necessary, you can bring one person to help you. We want to make sure it does not get too
busy at the vaccination centre.
How long does the appointment take?
The appointment takes about half an hour. Please be at the vaccination centre about 5 minutes before your appointment.
First, staff will ask you a few questions. Then you will get the vaccination. You have to stay for 15 minutes afterwards so we
can see how you react to the vaccination.
What if you cannot come to the appointment?
Can you not come? Then you can move or cancel your appointment. You can do this using your DigiD on coronavaccinatie-
afspraak.nl. Go to

. You can also move the appointment by calling 0800-7070. Have your
citizen service number ready when you call.
What if we cancel the appointment?
We may have to cancel your appointment for some reason. In that case, we will let you know before the appointment.
Did you make the appointment for your child? Then this is important for you to know:
To make sure it does not get too busy, only one parent or carer is allowed to come.
Has your child had close contact in the last 5 days with someone who has coronavirus? Then your child may get
coronavirus. In that case, your child will not need a vaccination and you can cancel the appointment.
Both parents must give permission for the vaccination. As the parent, you are responsible for discussing your child

vaccination with the other parent. Staff at the vaccination centre will ask if you have done this. If one of the parents has
not given permission, we cannot vaccinate your child.
Are you getting a vaccination because you need it in another country?
You may be getting this vaccination because you want to travel to a country where you have to be vaccinated. In that case,
there is no medical need for the vaccination. This vaccination is at your own request. Do you want more information about

Are you getting this vaccination voluntarily (in other words: not for medical reasons)?
