1. Codice di identificazione unico del tipo di prodotto:

previsto dall’articolo 11, paragrafo 4:

dal fabbricante:

4. Denominazione, denominazione commerciale registrata o marchio commerciale registrato e
indirizzo di contatto del fabbricante, come previsto dall’articolo 11, paragrafo 5:

5. Se applicabile, nome e indirizzo di contatto del delegato il cui mandato prevede lo svolgimento dei
